Cancel Service

This form is to cancel your utility service with the City of Oregon City. When using this form, please make your cancellation date as close as possible to the date you actually turn your keys in.

Example : When the home keys have been turned over to the next responsible party.

Multiple Family Dwellings

If this service is for a duplex, tri-plex, or apartments, we require that the seller of the property provide new owner information in the comments section of this form.

Date the keys have been turned over to Owner, New Owner, Managment Co.

This field must be filled on, this date is required.

Name the Account is under or how it's to appear on the billing statement

Include state

Customer Number - located on monthly billing statement

Current or Vacating Address

Forwarding Address - where the final billing can be mailed

Phone at current address, Cell phone # if there is no home phone

Phone Alternate - after move

*Renters must supply either Social Security Number or Drivers License Number.

*If Checked Duplex, Triplex information regarding the new owner must be supply by the person stopping the service.
